* Offer scholarships of excellence with a desire to participate in volunteering in the community and in extra-curricular activities sponsored by the CNDPS and the CNDPS-AA.

* Applicants must have a minimum GPA of 8.0.

* Scholarship recipients must belong to a low-income family.

* The scholarship funds will be paid in (XXX / XXXX) for the first year of the student directly to the CNDPS and not to the student. The school will be responsible for providing a student progress report and bill for the next school year.

* CNDPS-AA will only accept ### applications per year. Applicants must have CNDPS approval certifying that they are qualified for this scholarship program. The registration deadline is XXX / XXXX

Applications will be reviewed and recipients will be selected by a committee composed of Board Members of the Association and College Notre Dame du Perpetuel Secours. The scholarships will be awarded in XXX / XXXX.

The basic process for evaluating candidates and awarding scholarships is as follows:

STEP 1:Three Standard Selection Criteria: All applicants are evaluated by evaluators according to the following three standard criteria:

1. High academic achievement. This is based on your academic transcript, and the testimony of your academic excellence that is provided by a letter of recommendation from a teacher.
2. Financial Need: This is based on the financial information that is provided by your letters of recommendation, plus any reinforcement narrative you provide.
3. Contribution to the community: This is based on the information presented by your letters of recommendation, as well as any reinforcement stories you provide.

STEP 2: Decision-making: Decisions regarding which specific scholarships are awarded are based on the evaluation of the information gathered. Donors applying to participate in the final decision for their particular awards are part of the decision.

STEP 3: Scholarship Confirmation: Once all conditions are met, scholarship recipients will be officially notified by email of their scholarship. As a courtesy, candidates who fail in the scholarship program will also be notified by email.

STEP 4: Scholarship Award Ceremony: This ceremony is scheduled for _______________ and will take place at the CNDPS, Cap-Haïtien, Haiti. All fellows and their families and friends will be invited. A candidate's inability to attend the awards ceremony in person will have no effect on his or her eligibility for a scholarship.

STEP 5:There will be an obligation to fulfill certain civic duties, such as: institutional and community sanitation, tutoring for those with academic difficulties, etc.

To be admitted to the CNDPS-AA scholarship program, you must meet the following criteria:

• Attend College Notre Dame du Perpetuel Secours 
• Maintain a minimum GPA of a 8.0
• Demonstrate need for financial assistance
• Be prepared to offer a minimum of 8 hours per month in one of the various volunteering programs identified by the institution to include tutoring services for those whose with less than excellent academic performance 

Candidates must demonstrate their desire and ability to overcome obstacles and achieve their goals. Applications will be evaluated on:

• Individual determination to succeed
• Future goals and plans to achieve goals
• Ability to communicate difficulties you have overcome or are currently facing
• Self-motivation in carrying out difficult courses

CNDPS-AA will partner with various entities from various sectors to encourage participation one of the following activities:

• Internship (hospital, radio, agriculture etc.)

• Health Awareness (Human Immunodeficiency (HIV), Sexual Disease transmissible (STD), malaria, etc.)

• Tutoring

• Sanitation (street cleaning etc.)
